Job Summary
As a Licensed Physical Therapist with our organization, you will provide office support for scheduling, physician communications, and care coordination, enabling you to concentrate on providing exceptional care for your patients in-home settings.
Responsibilities
- Provide occupational therapy services to home care patients and engage in primary case management when required.
- Maintain timely and accurate communication and documentation related to occupational therapy delivery.
- Offer clinical supervision to Certified Occupational Therapist Assistants.
- Promote and develop occupational therapy services within the home care community.
Requirements
- Two years of experience in home care.
- Demonstrated ability to multitask.
- Proficiency in working and communicating effectively with patients, medical partners, employees, and supervisors.
- Able to work independently and as part of a team.
- Graduate from an accredited Occupational Therapy School.
- MN OT license prior to employment.
- CPR certification (healthcare provider).
- Technological proficiency and a commitment to accuracy.

Schedule
Day Shift, Monday through Friday, business hours 8:30am-5pm, with participation in the on-call rotation.
