Location : Hiring for the South Metro (Minnetonka, Prior Lake, Bloomington, Eagan, Lakeville area) and the East Metro (Roseville, Cottage Grove, Stillwater, Forest Lake area)

Primary Responsibilities :
Provides Physical Therapy interventions under the supervision of a Physical Therapist to restore function relieve pain, improve function, and prevent disability to facilitate quality patient care. Complies with the direction of a Physical Therapist and applies therapeutic principles appropriately to provide competent patient care.
Specific Job Duties - includes, but not limited to :
Establish a rapport with patient and community in a professional manner.
Develops and documents effective home exercise programs and supplemental instructional programs as needed.
Communicates with the Physical Therapist as needed regarding patient progress and need for goal modification.
Provide documentation that justifies medical necessity and skilled need.
Knowledge of physical therapy principles; interpersonal dynamics; community resources; legal limitations within the practice of physical therapy.
Skilled at assessment and knowledgeable of growth and development.
Provide appropriate care respective to the ages of the patients served per clinical environment.
Attends on-the-job and in-service training.
Attends clinical and all other meetings accordingly.
All other duties and tasks as assigned
Experience :
2 Years of PTA experience (Preferred)
1 Year of Home Care Experience (Preferred)
Demonstrated ability to multi-task.
Ability to work and communicate professionally with patients, medical partners, employees, and supervisors.
Ability to work independently and as part of a team.
Required :
Graduate of an accredited physical therapy assistant program
Current Minnesota PTA certificate
Current BLS / CPR Certification
Good judgement and decision-making skills.
Strong attention to detail.
Technologically proficient and driven by accuracy.
Provide dependable transportation for patient visits.
Act as a role model of Advanced Medical Home Cares values and mission.
Regular attendance and flexibility with scheduling necessary.
Working Conditions :
Ability to sit, stand, type, see, hear, communicate verbally and in writing.
Ability to drive a vehicle.
Ability to complete job tasks within 32-40 hours per week as stated in offer.
Contact with patients under wide variety of circumstances.
May be exposed to patient elements, unpleasant elements (accidents, injuries / illness, pets, insects, communicable diseases) and may be subject to varying and unpredictable situations.
Handles emergency or crisis situations
