WANTED:
- 1-2 years of experience in the world of long-term care—bonus points if you're a pro!

REPORTING TO:
The Social Services Designee will report directly to the Director of Social Services—think of it as the social superhero chain of command!
YOUR ROLE:
- Jump right into those assigned committees like a pro!
- Take the reins on discharge planning while staying in the good graces of California Title 22 and Federal rules.
- Team up with HMOs, residents, families, DME suppliers, Home Health Agencies, transportation services, and Adult Protective Services to ensure everyone gets home safely.
- Be the mastermind behind a transportation system that meets all off-campus resident adventures!
- School residents, families, and staff on their rights like a rights rockstar, according to California Title 22 and Federal regulations.
- Dive deep into comprehensive psychosocial assessments, following the Social Services playbook.
- Keep those progress notes and care plans updated faster than a caffeinated squirrel!
- Bring residents and families into the care planning process and cheer them on during care conferences.
- Show up for all the mandatory in-service training sessions – because learning is a lifelong party!
- Help residents and families navigate the ups and downs of moving into a nursing center like the supportive champion you are.
- Manage room changes, bed assignments, and keep the convo flowing with residents, roommates, and responsible parties.
- Plan discharges with residents and families to ensure smooth sailing during transfers and discharges.
- Book appointments with the Ombudsman and families for Durable Power of Attorney for Health Care like a scheduling wizard!
- Host family conferences whenever the need arises – let’s get everyone on the same page!
- Make sure everyone is on track with Advance Directives by following up with residents, families, and/or Power of Attorney.
- Conduct an annual rights review session with residents, families, and/or Power of Attorney – let’s keep those rights fresh!
- Take on extra tasks from the Director of Social Services or Administrator as they come your way!
