Onsite Rehab and Nursing Care, Inc., Sc in Lincolnwood, IL is looking for one quality assurance home health to join our strong team. We are located at 7301 N Lincoln Avenue Ste 185. Our ideal candidate is attentive, ambitious, and hard-working.
Responsibilities
1. Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing.
2. Performs comprehensive review of assessments and evaluations of client status, including physical, psychosocial, and environmental parameters.
3. Completes comprehensive review of assessments including OASIS data set within forty-eight (48) hours of referral for speech therapy unless other arrangements are made.
4. Monitor compliance of an individualized Plan of Care, that incorporates data analysis and current scientific findings.
5. Provides skilled interventions and mentorship with a focus for achieving realistic client outcomes within a specified time period.
6. Reports, communicate, and suggest changes of relevant information necessary to modify and update the care plan to reflect progress towards goals (outcomes).
7. Consistently demonstrates competency with technical nursing skills according to the personal and legal scope of practice:
8. Teaching and training skills according to the client's needs and within the context of the client's environment, value system, and physical/emotional status.
10. Medication management.
11. Practices nursing with respect for individual, cultural, and spiritual differences.
12. Communicates effectively and in accordance with Agency requirements.
13. Reports significant changes in client status to the physician and other members of the team in 14. timely manner consistent with client needs.
15. Maintains client records showing systematic assessment, planning intervention, and evaluation.
16. Participates in team meetings and clarifies individualized reporting responsibilities with all new assignments.
17. Documents legibly and according to Agency documentation guidelines and standards.
18. Follows Agency guidelines for completion of forms, reflecting the orders and care plan.
Submits forms weekly-including all communication and coordination.
19. Initiates actions to improve the quality of patient care. Takes appropriate actions independently and responsively in- home care situations. Initiates referrals as needed to other health team members when delegated by the Case Manager.
20. Maintains confidentiality in all aspects of the job.
21. Promotes personal safety and a safe environment for clients and coworkers.
22. Demonstrates knowledge of safety/infection control practices by complying with established policies and procedures.
23. Recognizes and responds appropriately to potentially unsafe situations.
24. Demonstrates safe practice in the use of equipment.
25. Assesses environmental safety and takes initiative to prevent accidents and promote safety.
26. Notifies supervisor of need for review and/or training related to equipment, procedures, or safety/infection control practices.
27. Participates in ongoing, professional self-development.
28. Participates in all mandatory education per Agency guidelines.
29. Identifies needs for personal and professional growth.
30. Demonstrates competency in areas of assignment or identifies the need for additional training (pediatrics, infusion, specialized equipment, etc.).
31. Supervises applicable home care team members as delegated by the RN Case Manager. I 0. Performs other duties as assigned.
Qualifications
Graduate of an accredited school of professional nursing. BSN, PHN preferred.
• Current license as a Registered Nurse or LPN in the state(s) of practice.
• Minimum of one (I) year experience in a Home Health Agency setting or equivalent experience.
• Current CPR certification.
• Strong communication and interpersonal skills.
