Job Summary:
The Community Health Liaison II will act as the primary point of contact between client outpatient programs and Advantage Healthcare Services pharmacies. This role is crucial in ensuring seamless coordination of medication management, enhancing patient outcomes, and supporting the overall mission of both organizations. The CHL II will work closely with healthcare providers, patients, case managers and pharmacy staff to streamline communication, resolve issues, and provide exceptional service. The CHL II will work on AHCS Enhanced Clinical Service and Support Service Programs, as part of the Clinical Pharmacy Team.
Key Responsibilities:
- Serve as the main liaison between program staff, patients, and Advantage Healthcare Services pharmacies.
- Coordinate medication management, ensuring timely and accurate dispensing of medications.
- Facilitate communication between healthcare providers and pharmacy staff to optimize patient care.
- Monitor and track medication deliveries, addressing any issues or delays promptly.
- Implement and manage adherence packaging solutions to support patient medication compliance.
- Provide integrated reporting solutions to manage delivery failures and ensure medications are re-routed appropriately.
- Educate Program staff and patients on pharmacy services, adherence packaging, medication management best practices and AHCS Clinical Services
- Assist in the development and implementation of pharmacy-related policies and procedures.
- Maintain comprehensive records of all interactions and transactions related to pharmacy services.
- Support the goals and mission of Advantage Healthcare Services and the Outpatient Mental Health Organization by enhancing the quality of care provided to patients.
- Conduct Enhanced Clinical Services to AHCS patients
- Conduct in person patient visits for enhanced services programs at partner organizations.
- Communicate and coordinate care with patients' healthcare provider and care team.
- Optimize outcomes in patient adherence and access to prescriptions.
- Attend meetings and work with other clinical team members.
- Complete required reporting requirements in a timely manner.
Qualifications:
- Pharmacy technician certification or related qualification preferred.
- Minimum of 2 years of experience in a healthcare or pharmacy setting.
- Strong understanding of medication management and pharmacy operations.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with healthcare providers, patients, and pharmacy staff.
- Strong organizational skills and attention to detail.
- Proficiency in using healthcare information systems and software.
- Experience or Proficient in Microsoft 365 or Microsoft Outlook
- Commitment to providing exceptional service and support to patients and staff.
- Ability to travel to Sacramento on occasion as required.
Working Conditions:
- Full-time, typically requiring 40 hours per week
- Work is performed in an office setting or on the road
- Prolonged periods sitting at a desk and working on a computer
- Must be able to lift up to 15 pounds at times
- Able to travel as needed
