Job Description
The Service Coordinator plays a critical role in ensuring the success of our home care services. Key responsibilities include coordinating with clients, staff members, and community resources to deliver exceptional customer service, maintaining accurate records, and communicating effectively with the Branch Manager.
Required Skills and Qualifications
- Valid state-issued driver's license required.
- Develop and maintain positive relationships with clients, staff members, and community resources.
- Deliver high-quality customer service to clients and field staff, addressing requests in a timely manner.
- Stay informed about state, federal, and business regulations.
- Communicate effectively with Branch and Corporate staff.
- Coordinate onboarding process, complete new hire training, and participate in in-service training.
- Timely communication with new client referrals and held clients to coordinate care.
- Collaborate with the Service Coordinator to meet branch staffing needs.
- Collect, review, and submit documentation accurately and efficiently.
- Maintain accurate medical records and order office supplies.
- Address problems and complaints with the Branch Manager and guarantee confidentiality.
- Participate in special projects and attend in-service trainings as needed.
