The ideal candidate will be comfortable collaborating with a multidisciplinary team, effectively communicating with families, and ensuring high-quality patient care.
Key Responsibilities :
- Develop and implement individualized treatment plans to enhance patients' functional abilities.
- Work collaboratively with other healthcare professionals to ensure a comprehensive approach to patient care.
- Communicate effectively with patients, families, and caregivers to support therapy goals.
- Maintain thorough and accurate documentation using EMR systems.
- Provide cognitive rehabilitation therapy, particularly for individuals with behavioral disturbances associated with Dementia.
- Deliver Dementia sensitivity training and assist in program development for memory care units.
- Treat patients in assisted living facility settings, with training and mentorship available for those new to this environment.
