Quality Assurance Specialist - Hospice
Advantage Hospice is seeking a detail-oriented and experienced Quality Assurance Specialist to oversee and enhance our quality initiatives across hospice care services. This role ensures compliance with local, state, and federal regulations, improves performance, and supports the highest possible quality of care for patients and families.
Key Responsibilities:
- Conduct site-specific audits of hospice quality measures, providing comprehensive reports and actionable findings.
- Present audit results to leadership, assist in developing plans of correction, and integrate performance improvement projects with Quality Assurance Performance Improvement (QAPI) initiatives.
- Educate and guide local leadership on CMS, State, and accreditation standards of practice.
- Perform mock surveys consistent with state and federal guidelines to ensure sites are survey-ready year-round.
- Compile post-survey reports and collaborate with local leadership to implement corrective action plans to maintain compliance.
- Monitor team adherence to policies, procedures, and regulatory requirements at all levels.
- Collaborate with the Clinical Director and Supervisors to ensure policies and procedures support the highest standards of patient care.
- Analyze and report quality data, including patient care, safety, risk management, infection control, outcomes, and patient satisfaction trends.
- Actively participate in Quality Assurance Performance Improvement initiatives, including chart audits and strategic planning.
- Maintain up-to-date knowledge of Medicare Conditions of Participation and accrediting organization standards to ensure compliance.
- Attend and contribute to staff meetings, department meetings, in-services, and other related activities.
Qualifications:
Education: Graduate of an accredited school of nursing; BSN preferred.- Licensure: Valid Registered Nurse (RN) license in Pennsylvania.
- Experience: Minimum of 3 years of hospice experience required.
- Skills: Strong analytical, organizational, and communication skills. Ability to lead quality initiatives and work collaboratively across teams.
