In this role, you will be responsiblefor conducting in-home wellness risk adjustment assessments forMedicare members and other at risk populations. Your primaryobjective will be to assess the overall health and well-being ofmember beneficiaries to ensure accurate and comprehensive riskadjustment coding, leading to greater value-based care. Learn more about ourprimary purpose here Follow us on LinkedIn : https : / / www.linkedin.com / company / advantmed NP Responsibilities : Perform annual in-home wellness visits and risk adjustment healthassessments on a population with chronic conditions; Our thoroughevaluations include : a comprehensive assessment, physicalexamination, body system review, vital signs, review of medicationhistory, review of current prescriptions, review of memberfunctional status, pain management, cognitive assessment, review ofthe home environment, fall risk screening, social assessment,review of daily living activities, mental health screening,documentation and communication​ You will play a pivotal role inproviding individualized quality care to the elderly population intheir homes Deliver patient health education on topics such as painmanagement, medication, etc. Build relationships of trust withmembers through exceptional communication and empathy Assist inclosing quality care gaps (i.e. screenings and labs) Anenthusiastic collaborator contributing to the enhancement of caredelivery Providers are expected to commit a minimum of 30 hours permonth Locations : Douglas County, OR Requirements NP Qualifications : Must have a valid unencumbered NP License for the state you will beworking in This role requires travel within the assigned jobposting county up to a maximum 55-mile radius Previous in-home riskassessment experience preferred Previous 1099 (PRN) experience ispreferred but not required 3 years patient care experiencepreferred (primary care / adult / geriatric, EMR) May be requested toobtain additional NP licensure supported by Advantmed Access toreliable transportation that will enable you to travel to member'shomes within a designated area Strong ability to work within ourEMR system Ability to work independently Bilingual is a plusBenefits Advantmed offers : Competitive wages (contractor per diem,per completed in-home assessment rate $130) Paid mileage Flexiblework schedule Evening and weekend availability No on-call Visitsready to be scheduled immediately Appointment confirmation supportDedicated coordinator support Advanced member scheduling coverageState of art technology
