Job Summary
We are seeking a highly skilled and compassionate Physician Assistant (PA) to join our Advantmed provider network.
In this role, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare and other populations.
Main Responsibilities
- Perform Annual Wellness Visits and Health Assessments
- Deliver Patient Health Education Opportunities
- Assist in Closing Quality Care Gaps
Requirements
- NP Qualifications: Must have a valid unencumbered NP License for the state you will be working in
- Previous Experience: Preferred 3 years patient care experience (primary care/adult/geriatric, EMR)
- Bilingual: A plus
