### Key Responsibilities:
- Conduct annual in-home wellness visits and risk adjustment health assessments on populations with chronic conditions
- Perform thorough evaluations, including comprehensive assessments, physical examinations, body system reviews, vital signs, medication history reviews, prescription reviews, functional status reviews, pain management discussions, cognitive assessments, home environment reviews, fall risk screenings, social assessments, daily living activity reviews, mental health screenings, and documentation
- Provide individualized quality care to elderly populations in their homes
- Deliver patient health education on topics such as pain management, medications, and other relevant health issues
- Build relationships of trust with members through exceptional communication and empathy
- Assist in closing quality care gaps, including screenings and lab work
- Collaborate with healthcare teams to enhance care delivery
### Requirements:
- Must have a valid unencumbered NP License for the state you will be working in
- This role requires travel within the assigned job posting county up to a maximum 55-mile radius
- Previous in-home risk assessment experience is preferred
- Previous 1099 (PRN) experience is preferred but not required
- 3 years of patient care experience, preferably in primary care, adult, or geriatric care, with EMR proficiency
- May be requested to obtain additional NP licensure supported by Advantmed
- Access to reliable transportation for traveling to member homes within a designated area
- Strong ability to work independently and within our EMR system
- Bilingual skills are a plus
