Responsibilities
This role involves conducting in-home wellness risk adjustment assessments for Medicare and other populations. The successful candidate will also be responsible for delivering patient health education opportunities and assisting in closing quality care gaps.
Requirements
- NP Qualifications: Must have a valid unencumbered NP License for the state you will be working in
- Previous in-home Risk Assessment experience preferred
