Case Manager
Overview:
We are looking for a dedicated and empathetic Case Manager to join our team. As a Case Manager, you will play a crucial role in coordinating care and support services for patients in need. The ideal candidate will excel in geriatrics, Asssited living patient assessment, Care plan, nursing care, and evaluation.
Duties:
- Conduct comprehensive patient assessments to determine individual needs and develop personalized care plans.
- Collaborate with healthcare professionals to ensure seamless coordination of patient care.
- Provide support and guidance to patients and their families throughout the treatment process.
- Monitor and evaluate patient progress regularly, adjusting care plans as necessary.
- Advocate for patients' rights and ensure they receive appropriate medical services.
- Maintain accurate and confidential patient records in compliance with healthcare and COMA regulations.
- Provide direct nursing care to residents, including assessments, medication administration, and care planning.
- Act as the primary point of contact for residents and their families, ensuring effective communication about health status, treatment plans, and changes in condition.
- Supervise and delegate tasks to nursing assistants, and other caregiving staff while ensuring adherence to facility policies and regulatory requirements.
- Develop and implement personalized care plans based on resident assessments, medical history, and individual needs.
- Monitor and evaluate the health status of residents, making necessary adjustments to care plans and collaborating with physicians as needed.
- Administer prescribed medications and treatments in compliance with legal and ethical standards.
- Ensure proper documentation of care provided, medications administered, and changes in residents’ conditions.
- Monitor residents for signs of potential health issues, including changes in mental and physical status, and provide appropriate interventions.
- Educate residents and their families on health management, disease prevention, and self-care practices.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to provide holistic care.
- Ensure that residents' rights and dignity are respected at all times in line with the facility’s values.
- Participate in quality improvement initiatives and assist with maintaining regulatory compliance.
- Provide on-call nursing support when necessary.
Qualifications:
Experience:
- Proven experience as a Case Manager or similar role in healthcare settings.
- Strong background in hospital medicine, geriatrics, behavioral health, acute care, or emergency medicine.
- Proficiency in conducting patient assessments and developing care plans.
- Knowledge of diagnostic evaluation techniques and treatment modalities.
- Ability to work collaboratively in a fast-paced environment while prioritizing tasks effectively.
