About the Role:
- Develop and implement case management strategies to ensure efficient and effective discharge planning.
- Collaborate with interdisciplinary teams to achieve hospital metrics and regulatory compliance.
- Ensure accurate and timely documentation of case management activities.
The Ideal Candidate:
- Minimum 3 years of direct Case Management/Utilization Management experience in an acute care setting.
- 2 years of Manager-level (or above) experience in Case Management Leadership.
- RN or MSW degree required.
- Strong leadership and communication skills.
