Job Overview
We are seeking a dedicated and compassionate Case Manager to join our healthcare team. The ideal candidate will have a strong background in social work and experience in a hospital setting, particularly within the Pediatric Intensive Care Unit (PICU) or emergency medicine. As a Case Manager, you will play a crucial role in coordinating patient care, ensuring compliance with HIPAA regulations, and facilitating access to necessary medical resources. Your expertise in diagnostic evaluation and addiction counseling will be essential in providing comprehensive support to patients and their families.
Responsibilities
- Conduct thorough assessments of patients’ needs and develop individualized care plans.
- Coordinate with healthcare providers, social workers, and families to ensure seamless patient services.
- Review and manage medical records while ensuring adherence to HIPAA regulations.
- Facilitate diagnostic evaluations and follow-up care for patients in the PICU or emergency settings.
- Provide addiction counseling as needed, connecting patients with appropriate resources.
- Monitor patient progress and adjust care plans as necessary to meet evolving needs.
- Engage in utilization management to optimize resource allocation and improve patient outcomes.
- Advocate for patients’ rights and ensure they receive the highest quality of care.
Experience
- Bachelor’s degree in Social Work or a related field; Master’s degree preferred.
- Previous experience in a hospital environment, particularly within PICU or emergency medicine settings.
- Familiarity with medical records management and HIPAA compliance is essential.
- Strong skills in diagnostic evaluation, addiction counseling, and utilization management are highly desirable.
- Excellent communication skills, both verbal and written, with the ability to build rapport with patients and families.
- Proven ability to work collaboratively within interdisciplinary teams to achieve optimal patient outcomes.
