The role you’ll contribute:
To utilize the nursing process in the direction and administration of the medical and nursing plan of care. This performance is in compliance with established structure standards, process standards and protocols, under the supervision of the Sr. Nurse Manager Behavioral Health and Director of Inpatient Nursing.
The Behavioral Health Care Coordinator Nurse will ensure the delivery of safe and effective patient care as defined by the Advent Health East Division standards of nursing care and is consistent with the therapies of other disciplines.
The Behavioral Health Care Coordinator Nurse functions under the Behavioral Health Unit (BHU) and reports to the Behavioral Health Senior Nurse Manager. The Behavioral Health Care Coordinator Nurse is a highly responsible and specialized direct service clinical worker functioning as a care coordinator serving a variety of populations who have mental and substance abuse issues. The Behavioral Health Care Coordinator Nurse works in the BHU and serves as a liaison between the BHU and referring facilities to coordinate admissions and care of behavioral health patients.
Work involves screening and assessing patients for Voluntary admission to the BHU based on admission criteria established by the BHU, monitoring and tracking referrals made to the BHU, completing the admission process for patients accepted to the BHU, obtaining authorizations for services and treatment, and providing patient care as staffing and unit needs dictate.
The value you’ll bring to the team:
- Monitors the Involuntary/Voluntary Care and Medically Cleared report for potential referrals to the BHU and care coordination needs of patients with behavioral health concerns within the East Florida Division.
- Screens patients presenting to the Emergency Department (ED) or on the inpatient units who have mental health and/or substance abuse complaints for potential Voluntary admission to the BHU in accordance with DSM-V criteria.
- Provides referral resources to patients in the Emergency Department who do not meet criteria for admission to the Behavioral Health Unit (BHU).
- Documents and tracks all referrals to the BHU according to the unit procedure.
- Acts as a resource for behavioral health needs to ED staff, in-patient nursing units, Care Management, patients, and family.
Qualifications
The expertise and experiences you’ll need to succeed:
- Associates Degree in Nursing from an accredited school of nursing.
- One year nursing experience is preferred.
- One year of clinical mental health/substance use experience or related experience.
- Active Florida RN license is required
