The role you’ll contribute:
The Care Management Assistant is responsible for assisting in coordination of care and services for assigned patients under the direction of Care Management Leadership, and in collaboration with the multidisciplinary team and patient/family. The Care Management Assistant provides clerical support to the Care Managers and Social Workers to facilitate and execute discharge plans and other related activities, assists in responsibilities related to regulatory requirements, actively participates in outstanding customer service, escalates as appropriate to effectively and promptly resolve issues, and accepts responsibility in maintaining relationships that are equally respectful to all.

- Demonstrates appropriate customer-care skills such as empathy, active listening, courtesy, politeness, helpfulness and other skills as identified.
- Performs skills necessary to create a high-quality patient experience, as reflected through acceptable satisfaction scores, quality monitoring, and patient feedback.
- Coordinates and schedules post-acute follow up appointments with physicians, testing, and other services when appropriate.
- Copies, scans, files, and distributes documents.
- Documents activities and progress in the electronic medical record per defined policy and procedures or in other ways as directed. Inputs clinical information and discharge planning details into the medical record as appropriate for the role.
Qualifications
The expertise and experiences you’ll need to succeed:
- High School Diploma or GED
- At least 6 months of experience in a healthcare environment
- Experience working in a healthcare setting
