Role Description
This is a full-time on-site role for a Medical Social Worker located in Daytona Beach, FL. The Medical Social Worker will be responsible for providing social work services, discharge planning, case management, and social services to hospice patients and their families.
Qualifications
- Medical Social Work and Social Work skills
- Experience in Discharge Planning and Case Management
- Strong background in Social Services
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary team
- Compassionate and empathetic approach to care
- Master's degree in Social Work or related field
- Licensed Clinical Social Worker (LCSW) certification is a plus
