The value you’ll bring to the team:
- Provides independent advanced practice nursing care including but not limited to completions of assessment, diagnosis, development of treatment plan including outcome identification, patient/family education, and implementation and evaluation of the treatment plan.
- Performs patient rounding with an interdisciplinary team and participates in case conferences.
- Performs comprehensive physical, psychological, social, spiritual, and cultural assessments through observation, interview, review of medical records and clinical data inclusive of imaging and lab results.
- Evaluates patient’s response and intervenes to ensure optimal patient outcomes
Qualifications
The expertise and experiences you’ll need to succeed:
- Palliative care experience desired - inpatient or outpatient
- NP/PA Combined Hospice and Palliative Medicine 0-2 YOE $64.15/hr.
- NP/PA Combines Hospice and Palliative Medicine 3 YOE $68.66 /hr.
- Bachelor’s degree, Nursing- Required
- Master’s degree, Nursing- Required
- Basic Life Support- American Heart Association - Required
- Registered as Advanced Practice Nurse - Required
- Registered on the State of Colorado Advanced Practice Registry - Required
