Location : Manchester, KY Specialty : Family or Internal Medicine
AdventHealth Manchester is looking for a dynamic and energetic Physician to join our Skilled Nursing Facility provider team in beautiful Manchester, Kentucky (about 90 minutes southeast of Lexington).
The ideal candidate will be / have :
- Provide services within scope of a family or internal medicine physician; geriatric / SNF experience preferred
- Examine patients, take medical histories, maintain patient records, identify health risk factors, prescribe medications, make referrals, and otherwise treat health conditions
- Provides direct patient care within a skilled nursing facility setting in conjunction with APRNS who manage acute visits and assist with chronic visits. Timely physician-required visits to be done per CMS guidelines
- Focus on patient wellness and disease prevention, prevention of hospital readmissions, antibiotic stewardship, advance care planning, collaboration with families and rehab service providers.
- Provide health education services and / or counsel patients on how to make healthy lifestyle choices
- Risk Management : understand importance of documenting and addressing concerns such as risk of falls, malnutrition, polypharmacy, and pressure injury.
- Understands and complies with the Environment of Care / Infection Control Responsibilities
- Is knowledgeable and complies with the States Institutional Activities and Expectation
- Assists medical team with coordination of clinical activities and orders
- Licensure as a Medical Provider in the Commonwealth of Kentucky
