The role youll contribute :
The Patient Care Coordinator provides continuous and well-rounded care to surgical and non-surgical patients. Educates the patient and / or family on pre-operative and post-operative care in addition to the treatments being provided in the practice. Schedules surgical procedures, pre-admission testing and serves as a liaison between the patient and practice support staff. Monitors the patients' course of treatment and communicates pertinent information to the physician to obtain referrals and / or orders for medication and / or diagnostic tests. Displays a caring and professional attitude at all times as well as maintaining personal and professional growth.

Answers phones, schedules patient appointments and triages patient phone calls. Returns telephone calls in a timely manner.
Prioritizes work tasks, maintains a steady workflow, follows through until task is complete. Uses appropriate problem solving and decision-making abilities based on established protocols.
Thoroughly explains procedures, treatments, and nursing care to patients and / or family.
Reviews patient charts to ensure that all test results and correspondence are present prior to the physician seeing the patient. Obtains appropriate authorizations from patients insurance carrier.
Reviews appointment bookings and ensures preferences are given to patients in emergency situations. Maintains timely flow of patients.
Maintains and updates physicians schedule coordinating patient procedures. Schedules patient procedures and provides hospital information with applicable.
