The role you’ll contribute:
The RN Patient Care Coordinator is responsible for coordinating the care of patients within their specialty. Responsible for ensuring the full continuum of patient care is being met.

- Coordination: May be Responsible for the following:
- New patient consults and patient work up coordination
- Appointments and scheduling with continuation of care
- Patient Care: May be Responsible for the following:
- Initiates, facilitates and monitors the development of patient care pathways that identifies critical factors for achieving desired outcomes for discharge, post-hospitalization recovery, and health maintenance and promotion for a specific patient group.
- May assist in donor search, selection and coordination
- Communication:
- Promotes an environment of open communication as demonstrated through interaction with patients, staff, providers, faculty, administration and outside facilities.
- Utilizes positive problem-solving skills and conflict resolution to assess and diffuse difficult situations.
The expertise and experiences you’ll need to succeed:
Minimum qualifications :
- Associate’s Degree in Nursing (ASN)
- 1 year of RN experience
- Basic Life Support
- Florida State RN License
Preferred qualifications:
- Care Coordination Experience
- Relevant specialty experience
- Bachelor’s Degree in Nursing (BSN)
