Job Summary
Coordinates and oversees comprehensive services for structural heart patients throughout care continuum from referral through follow-up post procedures. Functions in multidisciplinary team as an advocate, interpreter, educator, clinical liaison, counselor and collaborator for patients and caregivers. Coordinates all disciplines involved in the care of the patient to ensure appropriate cost effective/efficient progression through managed care.

Job Requirements
Education and Work Experience:
- Bachelor's Degree in Nursing (BSN): Preferred
- Master's Degree in Nursing, Clinical Nurse Specialist or Nurse Practitioner: Preferred
- Acute care nursing experience: Preferred
- Cardiovascular nursing experience: Preferred
- Registered Nurse (RN) licensure in the state of practice: Required
- Advanced Cardiac Life Support (ACLS OR HS-ACLS OR RQIACLS) certification: Preferred
- Identifies and coordinates care for patients with heart valve disorders. Identifies individuals and recruits appropriate patients into program. Provides symptom assessment and nursing management, education, care coordination, and resource allocation as identified by continuous needs assessments. Receives and reviews referrals for patient appropriateness.
- Coordinates diagnostic workup by determining which tests need to be completed and assisting patients with navigating the organizations departments.
- Assists in providing volume and quality reports, tracking trends and identifying quality improvement initiatives. Assists in managing the valve data collection/registry. Assists in providing volume and quality reports, tracking trends and identifying quality improvement initiatives.
- Tracks identified navigational and quality metric, as well as implementation of interventions, to attain and maintain set of metrics. Demonstrates clinical expertise and assessment in the management of cardiac patients, within the scope of professional practice.
- Conducts in-services and educational activities with staff, as needed. Keeps abreast of latest technologies in heart valve repair and integrates best practices. Acts as a key contact with patients by coordinating care, providing patient education, and serving as the connection point with the organization.
