Job Summary:
Provides physician practices with support and consultation services about Substance Use Disorder (SUD) management for adults with varying levels of substance use and other behavioral health conditions. Helps primary care practices determine the next level of care for adults with substance use disorders and identify appropriate resources. Provides clinical care efficiently and effectively using a high level of technical and organizational skills as well as critical thinking ability.
Job Requirements:
Education and Work Experience:
- Bachelor’s Degree in social work, psychology or equivalent (some certifications will require Bachelor's): Required
- Master's Degree: Preferred
- Three years' technical experience: Preferred
- Two years’ Substance Use Disorder (SUD) assessment, treatment or related experience: Preferred
Licenses/Certifications:
- Licensed Clinical Social Worker (LCSW) or certification related to substance use disorders in state of practice: Preferred
Facility Specific License/Certifications:
- Licensed Clinical Social Worker (LCSW) in state of practice: Required
Essential Functions:
- Serves as the subject matter expert on Substance Use Disorder (SUD) resources, treatments and admission levels of care and criteria. Researches resources available for adults with behavioral health needs related to Substance Use Disorder, including outpatient individual or group therapy, detox, psychopharmacology, partial hospital programs, inpatient programs and self-help resources. Assists physicians and other providers in determining next level of care for patients with Substance Use Disorders. Provides patients with telephonic assessments to determine appropriate levels of care, when needed.
- Provides timely information turnaround to primary care practices and patients. Effectively triages and prioritizes requests based on patient presentation and problem severity. Communicates effectively with payers to identify accessible patient providers, taking into account coverage, geography and availability. Works collaboratively and effectively with primary care, social work, psychiatry, nurse practitioner and psychology personnel regarding referral services. Develops relationships with community-based mental health and substance abuse disorder providers. Identifies ways to better serve populations through programs.
- Develops expertise in available substance abuse disorder resources in and around partner networks. Performs referral specialist duties, as needed. Tracks and documents calls, turnaround times and outcomes. Manages and coordinates virtual conference aspects related to SUD care.
