Lying just inland between the State's coastal mountain ranges and the Sierra Nevada Mountains, Adventist Health Reedley has been serving the Central Valley since 2011. We are comprised of a 49-bed acute care hospital and 60 clinics in 27 rural communities with primary and specialty care services. Job Summary:
Provides support and assistance in making transition plans for individuals experiencing homelessness. Provides direction for routine aspects of non-medical problems. Assists with the planning and coordinating of services with other agencies to provide resources such as shelter, permanent housing, food, primary care doctors and all other identified non-medical related services. Assesses the patient's physical and emotional needs and arranges for services to meet those needs.
Job Requirements:
Education and Work Experience:
- Bachelor’s Degree or equivalent (some certifications will require Bachelor’s): Required
- Master’s Degree in social work or equivalent combination of education/experience: Preferred
- One year experience in an acute or long term medical setting: Required
- Two years' experience in an acute or long term medical setting: Preferred
- Two years' experience in homelessness coordination care: Preferred
Essential Functions:
- Initiates a plan of care. Reassesses plan of care according to the physical and emotional needs of patient.
- Develops Plan of Care (POC) based on patient assessment including, psychosocial factors that would impede the POC, clinical knowledge of appropriate theories of stages of human development, the impact of terminal illness, mental status and family dynamics.
- Utilizes effective problem solving strategies to reach decisions, take appropriate action and follow-through within assigned scope of responsibility.
- Obtains or coordinates resources from community, social, health and welfare agencies.
- Performs other job-related duties as assigned.
