- Completed graduate theological studies from an accredited seminary or divinity school.
- Experience in hospice care or a related field is highly preferred.
- A demonstrated ability to provide comfort and support to grieving patients and families.
- Strong listening skills and a patient-centered approach.
- Willingness to perform home visits and collaborate with the interdisciplinary team.
- Critical thinking skills to adapt spiritual care practices to individual patient needs.
Key Responsibilities:
- Conduct spiritual assessments to understand the unique needs of each patient and family.
- Develop personalized spiritual care plans in collaboration with local clergy if requested.
- Maintain compliance with all AdvisaCare policies, and applicable regulations.
- Provide spiritual care documentation in Electronic Medical Records accurately.
- Support team members in providing bereavement services as needed.
- Participate in team meetings and contribute to performance improvement initiatives.
- Uphold high standards of customer service and compassion in all interactions.
