Job Responsibilities:
- Interpret physician referrals and complete evaluations utilizing appropriate methods.
- Establish individualized treatment plans based on evaluation results and functional goals.
- Monitor and evaluate patient outcomes, adjusting treatment plans as needed.
- Communicate and collaborate with stakeholders to ensure optimum patient care.
- Document patient progress and maintain accurate records.
Requirements:
- Doctorate degree in Physical Therapy from an accredited school.
- Current Physical Therapy License in the State of Illinois or Wisconsin.
- Basic Life Support (BLS) certification.
- Excellent communication, interpersonal, and organizational skills.
