Job Description
Advocate Aurora Health is seeking a highly skilled Registered Nurse (RN) to fill the position of Home Health Case Manager - Infusion Therapy. This role involves providing optimal level of self-care for patients and quality of life through holistic assessment, plan of care development, and implementation.
- This position requires a Graduate from an accredited school of nursing with minimum 2 years Medical/Surgical experience and Critical care experience preferred.
- The ideal candidate will have good communication and interpersonal skills, as well as ability to determine appropriate frequency of visits.
- This position involves extensive travelling within geographical boundaries in all extremes of weather and exposed to hazardous materials with respect to patient care.
Responsibilities
- Holistic assessment on assigned patient according to established procedures including evaluation of home environment, availability of able and willing caregiver, identification of appropriate home care services, and reasonableness of expectation of patient's ability to achieve goals.
- Develops and adheres to appropriate visit frequencies for patient's plan of care.
- Implements plan of care/nursing care plan and revises accordingly.
- Minimization of patient/family knowledge deficits regarding disease process, patient/care, medications, infection control and specific procedures utilizing appropriate teaching processes.
This position offers an annual base salary of $74,672.00-$112,112.00 and is incentive pay eligible.
