About the Role
- Assume primary responsibility for assessment, planning, implementation, and evaluation of nursing care for assigned patients in the comfort of their own homes.
- Set goals that demonstrate a holistic approach to patients and families.
- Develop and maintain a therapeutic relationship throughout the healthcare continuum.
The RN will work closely with the team to coordinate medical needs and utilize resources such as social workers, CNAs, and MDs. Document and connect with the care team through point of care documentation.
