Schedule Details / Additional Information :
Typically Monday through Friday, 8am to 4 : 30pm. No on call or weekend requirement. Requirement for holiday rotation. Seeing approximately 6 patients a day. Area of responsibility / where patient care provided : Mequon, Thiensville, Bayshore, Whitefish Bay, River Hills, Brown Deer, Glendale and select zipcodes in north Milwaukee.
Major Responsibilities :
- Performs basic assessments, collects data, identifies problems and follows a care plan developed by the Registered Nurse.
- Administers medication and treatments. Provides wound care, hot / cold compresses, patches, G-tube, obtains specimens, takes patients vitals, and performs other medical procedures as documented in the plan of care.
- Communicates with Registered Nurse on the status of the patient after each visit and any changes in their patient's condition. Communicates any problems and documents the plan and actions taken in the patient's visit report.
- Follows up with the patient's physician as needed, including taking verbal orders.
- Participates in chart audits, Continuous Quality Improvement activities and in services as required. Records observations completely, accurately, legibly and concisely.
- Documents all information regarding the patients. Maintains and updates accurate clinical and patient records according to agency, state and federal guidelines.
- Communicates effectively with patient, family, or significant others and provides them with necessary emotional support.
- Performs various patient positioning and transporting duties, which require lifting and pushing / pulling, while utilizing proper technique.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his / her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
