About the Role
As a Registered Nurse in our Home Health team, you will be responsible for providing high-quality patient care and managing caseloads consisting of patients in specialty areas.
You will conduct initial assessments to obtain thorough physical, social, and environmental data, and establish plans of care in accordance with physician orders and patient needs.
Direct patient care and adjust nursing care processes as needed, identify problems, modify nursing care plans, and evaluate outcomes within an appropriate time frame.
Advise and consult with family and other personnel as appropriate, provide ongoing assessment of patient response to treatments and teaching, and apply interventions as necessary.
Complete documentation accurately and promptly, communicate with physicians to confirm and update patients' plans of care, and facilitate discharge planning according to standards.

- Demonstrate knowledge of clinical skills and the ability to work under direction and make sound judgments.
