- Conducting thorough assessments of patients' physical abilities and limitations

- Providing ongoing therapy sessions to patients in their homes or at local facilities

- Maintaining accurate records and documentation of patient progress and treatment plans

Requirements
To be successful in this role, you must have:
- A Doctorate degree in Physical Therapy from an accredited institution
- A valid Physical Therapy license in the State of Illinois or Wisconsin
- Basic Life Support (BLS) certification
- Strong communication and interpersonal skills
- Ability to lift up to 35 pounds without assistance
- Willingness to travel extensively within the geographical area and work rotating weekends and holidays
