Main Responsibilities
- Perform scheduled visits and collaborate with team members as necessary.
- Conduct thorough physical, psychosocial, and environmental assessments of assigned patients, updating plans of care where appropriate.
- Communicate with physicians to confirm and update patient treatment plans.
- Provide ongoing assessment of patient responses to treatments, medications, and teaching, applying interventions as needed.
Requirements
- Registered Nurse license issued by the state in which you practice.
- Basic Life Support (BLS) certification issued by the American Heart Association upon hire.
- Driver's license issued by the state of Wisconsin or Illinois.
