About the Position
This is an exciting opportunity to join our innovative Home Health team as a Nurse Navigator - Home Care. As a key member of our interdisciplinary team, you will be responsible for navigating patients through the healthcare system, ensuring seamless transitions between healthcare settings.
Responsibilities:
- Patient Navigation: You will work closely with patients, families, and healthcare providers to identify and address barriers to care, ensuring timely access to necessary services.
- Care Coordination: You will coordinate patient care across multiple settings, including hospitals, clinics, and home health agencies.
- Education and Support: You will provide education and support to patients and families on disease management, self-care techniques, and community resources.
- Communication and Collaboration: You will maintain effective communication with patients, families, and healthcare providers, promoting teamwork and coordination to achieve optimal health outcomes.
Requirements:
- Licensure: Current RN licensure is required.
- Experience: A minimum of 3-4 years of experience in nursing, preferably in home health or a related field.
- Skills: Excellent communication and interpersonal skills, strong critical thinking and problem-solving abilities, and proficiency in electronic medical records.
