What are the responsibilities and job description for the Director Community & Health Relations position at Advocate Aurora?
Major Responsibilities:
1)Direct the ACH community health needs in partnership with Governing Council members, Business Development, Planning, Mission and Spiritual Care, Finance and the Executive Council members when appropriate.
2)Solicit strategic partnerships in the community to develop and support health assessment and planning.
Lead the development of the ACH community health plan strategically responding to evidenced need.
1)Direct, coordinate, and link hospital specific resources and efforts to our Healthy Community Partnerships aimed at improving health status. Continue to expand the contribution in an effort to improve the quality of life of all community members.
2)Establish measurable objectives to meet community health goals, then develop and implement means of evaluation for the stated objectives; followed by the development of detailed action plans to meet stated goals.
3)Identify key demographic and health status indicators related to the health of the community, as shown by various county and state organizations, then devise methods to share these with community groups, clinical chairs, and hospital governance.
4)Achieves performance within organizational expectations. Coordinate community educational visits to the hospital, recognition and special events, and seminars, in conjunction with Public Affairs and Advocate Charitable Foundation, to increase hospital awareness and build relationships with various groups and policy makers.
5)Serve on hospital committees as liaison to community organizations and civic groups to better direct committee goals and objectives to ensure the effective and efficient implementation of community initiatives.
6)Eliminate duplication of services within the community and promote activities designed to stimulate other community organizations and individuals to join in carrying out a broad health agenda. To create outcome measures (re-administration of Health Needs Assessments) to help gauge progress toward goals set by each city’s Partnership.
7)Develop a civic culture within the community, by getting people to share in civic responsibility by bonding local governments with service organizations and various community groups through meetings/talks/forums and focus groups. Create healthy public policy through ongoing dialogue with individuals and city officials to understand the unmet needs of the community and work to fill gaps through initiating the change process and removing existing barriers.
Create and implement ACH local government strategies and plans in conjunction with site and system leadership.
1)Work with hospital’s President and Advocate’s Vice President of Government and Community Relations, to develop grassroots coordinating councils that will have a positive impact on health care in general, and particularly on ACH.
2)Work with hospital’s President to develop and administer communicative efforts aimed at local and state legislators to better inform them of our political, clinical and financial needs to enhance the hospital’s image and effectiveness.
3)Monitor and respond (while keeping the President informed) to legislative and policy developments in the design and execution of all local government activities.
4)Establish a professional, trusting relationship with local leaders by attending, as appropriate, city council meetings and local legislative activities and fund raisers.
Manage and coordinate community benefits.
1)Collaborate with Vice President of Finance to create, monitor and report quarterly community benefits that align with community needs.
Education/Experience Required:
Bachelors Degree, and 8-10 years experience in community or public health services. Experience in Public Health and/or Community Health. Masters preferred.
Knowledge, Skills & Abilities Required:
Highly proficient communication, motivational and presentation skills. Demonstrated experience in project and financial management, independent working skills, problem solving and team leadership. Demonstrated ability in “relationship building” in order to understand diverse needs of multiple stakeholder groups.
