The Director of Home Health Programs provides leadership and direction to Home Health programs and clinical operations teams including but not limited to Wound Care, Helping Hearts at Home, COPD, Standing Strong, Pediatrics, Infusion, weekends and any other programs designed to support Home Health.  Monitors and is responsible for the quality and metrics of the identified programs. Develops collaborative relationships across Continuing Health, both vertically and horizontally with regional stakeholders and home health partners. Directs the administration and coordination of multidisciplinary patient care services, including clinical, financial, operational, and personnel management to meet the goals and objectives of the organization in accordance with its standards, policies and regulatory requirements.

Major Responsibilities:

  • Develops and implements programs within the Home Health division that support the organizational goals and strategies. These programs may include but are not limited Wound Care, Helping Hearts at Home, COPD, Standing Strong, Pediatrics, Infusion, and the Weekend Program

  • Monitors and evaluates program performance.  Recommends and helps implement appropriate changes to leadership and stakeholders to achieve optimal results.

  • Provides leadership in planning, developing and providing high-quality cost-effective programs for home health patients

  • Develops, implements and oversees the metrics for home health programs. Facilitates regular meetings with partner

  • providers to ensure goals are being achieved. Implements quality plans for programs not achieving set metrics.

  • Develops collaborative and cooperative relationships with home health partners and leadership.

  • Establishes and maintains home care policies and procedures that are in full compliance with all appropriate local, state, federal and national accreditation body rules and regulations. Ensures that clinical areas and staff are in full compliance with such rules and regulations.

  • Monitors revenue and expenses for programs to assure adherence to budget. Develops and recommends operating and capital budgets and controls expenditures within approved budget objectives.

  • Implements and assesses clinical quality and patient experience initiatives to accomplish targets and ensures the highest possible delivery of service to patients, families, physicians and team members relative to their personal expectations.

  • Performs human resources responsibilities for staff which include interviewing and selection of new employees, promotions staff development, performance evaluations, compensation changes, resolution of employee concerns, corrective actions,

  • terminations, and overall employee morale.

  • Acts as a consultant and resource to staff.

Licensure, Registration, and/or Certification Required:

  • Registered Nurse license issued by the state in which the team member practices, or 

  • Physical Therapist license issued by the state in which the team member practices, or 

  • Occupational Therapist license issued by the state in which the team member practices, or 

  • Speech-Language Pathology license issued by the state in which the team member practices.

Will be required to be licensed in both Wisconsin and Illinois within six months of hire. 

Education Required:

  • Bachelor’s degree in nursing, or 

  • Bachelor’s degree in physical therapy, or 

  • Bachelor’s degree in occupational therapy, or 

  • Bachelor's Degree in speech pathology.

Experience Required:

  • ​​Typically requires 7 years of experience in clinical nursing or rehabilitation including experience in a home health care setting. Includes 3 years of management experience in managing staff and budgets. 

Knowledge, Skills & Abilities Required:

  • ​Must have strong verbal and written communication skills.

  • Must demonstrate and ability to organize, plan and delegate work effectively and efficiently.

  • Must demonstrate ability to resolve conflict, motivate staff and interact with superiors, peers and subordinates in a direct and professional manner.

  • Must possess the ability to solve problem and make independent judgments and decisions based on experience and good judgment.

Physical Requirements and Working Conditions:

  • ​​​​ Operates all equipment necessary to perform the job.

  • Position requires travel and thus there is exposure to road and weather hazards.

  • Must be able to sit approximately 70% of the workday.

  • Must be able to lift 50 lbs. occasionally.

  • Exposure to a normal office environment.

  • Occasional exposure to hazardous chemicals, loud noises or hazardous equipment.
