The MDS Nurse is responsible for coordinating and overseeing the completion of the Minimum Data Set (MDS) assessments and ensuring compliance with all regulatory requirements. This role ensures appropriate care planning and maximizing reimbursement through accurate documentation.
Key Responsibilities:
Complete and submit accurate and timely MDS assessments per federal and state regulations.
Coordinate interdisciplinary team meetings to develop individualized resident care plans.
Monitor and track assessment schedules to ensure compliance with regulatory timelines.
Analyze MDS data to support quality improvement initiatives and reimbursement optimization.
Educate staff on care documentation standards and regulatory changes.
Collaborate with nursing, therapy, and administrative teams to ensure resident-centered care.
Participate in audits, surveys, and inspections as needed.
Qualifications:
Current RN or LPN/RN license in MA
Minimum of [1-2] years of experience in long-term care or skilled nursing facility.
Prior MDS experience is preferred (MDS 3.0 knowledge is highly desirable).
Certification in Resident Assessment Coordination (RAC-CT) preferred or willingness to obtain.
Strong understanding of Medicare and Medicaid reimbursement processes.
Exceptional attention to detail, organization, and communication skills.
Ability to work collaboratively in a team environment.
