What are the responsibilities and job description for the Licensed Clinical Social Worker position at AdvocateMH?

The Licensed Clinical Social Worker plays a pivotal role in guiding patients to successfully connect with behavioral health resources and services. The Licensed Clinical Social Worker will work within a Family Primary Care Practice as the primary point of contact for patients with behavioral health needs, evaluating and triaging patients to an array of services and working in close coordination with a Collaborative Care Team. The Licensed Clinical Social Worker also works in tandem with a non-licensed Advocate who will tag in to assist in following up on the clinician’s treatment recommendations, and ensuring that patients are getting connected to services they need such as therapy, psychiatry, or support groups, online communities and free educational resources.  


Responsibilities

  • Conduct clinical evaluation of patient symptoms and concerns
  • Triage and make treatment recommendations
  • Provide education and guidance around options for care and resources 
  • Assist patients in locating well matched high quality resources to address presenting concerns
  • Collaborate with and provide clinical support to non-licensed Advocates interacting with patients about resources 
  • Coordinate care with referral sources as clinically indicated 
  • Offer patients continuous follow up and redirection to new resources as needed
  • Track outcomes of patient panel over time 
  • Partner with team on process improvement efforts 


Qualifications 

  • Master’s in Social Work and equivalent licensure in Massachusetts
  • Minimum of 3-5 years of experience of direct clinical practice in behavioral health
  • Previous experience managing clinical escalations and patient safety concerns 
  • Independent Social Work License in Massachusetts preferred
  • Previous experience working with patients with serious mental illness preferred
  • Comfortability and experience working with children and adolescents preferred 
  • Knowledge of the Collaborative Care Model a plus

  • Exceptional interpersonal and relationship building skills: ability to empathize and build trust and rapport with others easily
  • Excellent organizational skills: ability to manage multiple patient cases and required documentation concurrently
  • Adaptability and Flexibility: Willingness to adjust to changing priorities and demands. We are an early-stage startup that is growing very quickly. 


Working Conditions

This is an in person role with the majority of time spent working within the primary care practice though a portion of patient interaction with be telephonic
