About the Position:
We are seeking dedicated and compassionate IRIS Consultants to join our dynamic team at Advocates4U. As an IRIS Consultant, you will play a key role in empowering individuals to lead fulfilling lives by providing tailored support through the IRIS (Include, Respect, I Self-Direct) program.
IRIS Consultants will be knowledgeable in the IRIS program and follow IRIS regulations and guidelines. IRIS Consultants partner with individuals to keep them enrolled in the IRIS program to identify their long-term care goals and find creative ways to achieve them.
IRIS Consultants build upon the resources available in the community to help individuals develop customized plans to achieve their goals related to employment, housing, health, safety, community membership, transportation, and lasting relationships.
Key Responsibilities:
- Build Trusting Relationships: Establish and maintain strong, trusting relationships with individuals receiving services through the IRIS program, ensuring they feel supported and understood.
- Support Plan Development: Assist individuals in exploring how the IRIS program can provide the support and services necessary for them to lead more independent, fulfilling lives in the community.
- Navigating IRIS Requirements: Provide expert guidance to individuals to help them adhere to IRIS program policies and procedures. Maintain regular contact with individuals and meet documentation requirements.
- Create IRIS Support and Services Plans: Collaborate with individuals and their support network to develop personalized IRIS Support and Services Plans that meet their unique needs and goals.
- Quality Assurance: Consistently strive to meet and exceed quality standards, with a target of 100% in quality performance. Ensure that all documentation, plans, and communication are accurate, thorough, and meet organizational and program standards.
- Report to Supervisors: Regularly update and report to Supervisors on progress, challenges, and any areas requiring support. Take a hands-on approach in addressing issues and ensuring the successful implementation of the IRIS program.
- Continuous Learning and Improvement: Stay up to date with best practices and new developments in the IRIS program, sharing insights and innovative solutions with the team to enhance the lives of those served.
- Community Engagement: Strengthen community connections by fostering relationships between individuals served by IRIS and other community members, encouraging mutual support and inclusion.
- Team Collaboration: Work closely with other team members to foster a supportive work environment. Participate in hiring, peer monitoring, ongoing learning, and team-building activities to ensure mutual support and growth.
o Act as a resource for other IRIS Consultants in the IRIS Consultant Supervisor's absence.
o Provide program knowledge and support to other staff when appropriate, including, but not limited to reassignments.
- Attend mandatory trainings.
Qualifications:
- 4 years experience in social services, healthcare, or a related field, with a focus on supporting individuals with diverse needs.
- Possess a minimum of a Bachelor's Degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field.
- Pass a nationwide caregiver criminal history screening pursuant to the Department's policy (http://www.dhs.wisconsin.gov/caregiver/).
- Must possess a valid driver's license with current and adequate insurance for job-related travel.
- Must be willing to travel up to 2 hours as needed and comfortable working in homes with pets.
- Knowledge of the IRIS program/long-term care program and its requirements is preferred but not mandatory (training will be provided).
- Strong interpersonal and communication skills, with the ability to build rapport and trust with individuals and their support networks.
- Ability to work both independently and as part of a collaborative team.
- Passion for advocating for individuals' rights to self-direct their services and supports.
- A commitment to maintaining high-quality standards and meeting performance targets, ensuring that all work meets 100% quality expectations.
