Community Life Services has an immediate open for a full-time Licensed Practical Nurse, working in a residential setting with persons with developmental disabilities from . Immediate hire requires a valid AR driver's license, a current TB Skin test, and a current Arkansas Nursing License in good standing. Must be able to pass a criminal background check and pass a drug screen. EOE/M/F

Summary: Provides prescribed medical treatment to ill, injured, convalescent, and disabled persons in ICF/MR setting by performing the following duties.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Follow all doctors' orders as prescribed. Read entries written into consumer logs from previous shifts. Enter pertinent information into log prior to leaving shift.
  • Responsible for implementation of programs related to Self-Administration of Medication and appropriate documentation of such goals.
  • Take physician written/phone orders as need on scheduled shift and fax for signature as necessary.
  • Complete Nursing notes as necessary prior to completion of scheduled shift.
  • Complete Resident Leave packets for Home visits as needed.
  • Administer specified medication, orally or by subcutaneous or inter muscular injection, and note time and amount on patients' charts.
  • Monitor all medical charts for accuracy to include monitoring lab results and ensuring all orders are being followed as written.
  • Monitor and complete any follow ups necessary on Seizure and Marks reports.
  • Review and follow up on any recommendations submitted by medical consultants, with appropriate follow up charting.
  • Monitor and document weight, vital signs, blood sugar levels and diet for diabetic as needed.
  • Count narcotics weekly and sign book.
  • Order/maintain adaptive equipment when necessary as ordered by MD or recommended by team.
  • Report to Nursing Supervisor for clinical matters and report to QMRP/Director for all administrative matters.
  • Keep Nursing Supervisor informed of medical conditions/treatments of consumers.
  • Observe patients and report adverse reactions to medication or treatment to appropriate medical personnel.
  • Maintenance of supplies, including stocking and ordering new supplies and disposing of expired or damaged supplies.

Education and/or Experience: Licensed as a Practical Nurse in the state of Arkansas, Long Term Care experience and experience working with individuals with Developmental Disabilities preferred.

Certificates, Licenses, Registrations: LPN Certificate, Arkansas Driver's License, Auto Liability insurance, CPR/First aid certification, and TB skin test

Safety Sensitive Position: This is a safety sensitive position as it involves operating a motor vehicle as part of the job duties and a lapse of attention could result in injury, illness, or death to the consumers being transported. See Ark. Const. Amend. 98 2(25). All positions classified as safety must refrain from reporting to work or being subject to duty while their ability to perform job duties is impaired due to the use of alcohol or other drugs. All positions classified as safety sensitive are prohibited from testing positive for metabolites of substances that are listed in the schedules of the Federal Controlled Substances Act and the Arkansas Uniform Controlled Substances Act, regardless of whether the employee possesses a medical marijuana registry identification card. See id. at 3(f)(3)(B)(iii). This policy applies during all work hours, whenever conducting business for AEDD.

While performing the duties of this Job, the employee is regularly required to talk or hear and taste or smell. The employee is frequently required to use hands to finger, handle, or feel. The employee is occasionally required to stand; walk; sit; reach with hands and arms and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Work Environment: The noise level in the work environment is usually moderate. While performing the duties of this position the employee travels by automobile and is exposed to changing weather conditions.

Travel: Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected
