What would you do as a RN Case Manager (Home Health and Hospice) w / Flexible Schedules

As a new RN Case Manager in Home Health and Hospice at Aegis Healthcare, you will engage in a variety of responsibilities to ensure optimal patient care. Your day-to-day activities will include conducting comprehensive patient assessments, developing personalized care plans, and coordinating services with multidisciplinary teams. You will monitor patient progress, provide education to patients and families, and serve as their primary advocate throughout the healthcare process.

Additionally, you will document patient care accurately and maintain communication with all stakeholders to facilitate seamless care transitions. You may also participate in case conferences and collaborate with referral sources to enhance service delivery. Position Details :

• Status : Full-Time, Monday-Friday, 8 : 00 AM-5 : 00 PM
• Compensation : Full-Time RNs can earn $85K annually
• Work Area : East Valley (Mesa, Gilbert, Apache Junction)
