HEMODIALYSIS CHARGE NURSE – JOB DESCRIPTION

OVERVIEW OF THE JOB
The Hemodialysis Charge nurse is responsible for managing, supervising as well as providing administrative support and patient care.

Aside from managerial responsibilities, charge nurses perform administrative duties including creating schedules, maintaining adequate supplies and informing staff of changes to protocol.

MINIMUM QUALIFICATIONS
(Education, licenses, certifications, and experience required to fulfill the essential duties, include computer skills as required):

  • Licensed nurse in the state of assigned locations
  • Minimum one (1) year of nursing experience; minimum of six (6) months of hemodialysis experience
  • Fluent in the written and verbal English skills necessary to successfully perform all duties and responsibilities of this position.
  • Ability to be available to work during all hours of patient training and/or treatment
  • Possess valid driver’s license. Able to drive a vehicle and have reliable means to travel to assigned location. Driving record must meet insurance liability requirements.
  • Self-motivated and able to function independently
  • Passion for quality care, education and the individual team member’s appropriate role in the provision of care
  • Ability to motivate through influence and leadership
  • Ability to prioritize and adapt based on changing priorities
  • Ability to proactively identify and analyze clinical and financial outcomes
  • In-depth understanding of facility operations and the roles of direct caregivers
  • Visionary with ability to plan and execute projects that impact goals and objectives
  • Solid reasoning, critical thinking, problem solving abilities
  • Able to delegate appropriately
  • Detail oriented, yet able to drive projects forward
  • CPR certification desired

ESSENTIAL JOB FUNCTIONS

1. Accountable for the provision of outstanding customer service to all external and internal customers (skilled nursing(SNF) administration patients, staff, physicians)

2. Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner

3. Equipment and inventory management oversight to assure the safety of patients and staff
4. Independently identify, communicate and plan prompt corrective action through collaboration with SNF administration for any issues with building safety or equipment and systems
5. Participate in the development and periodic revision of policies and procedures with the Vice President of Clinical Operations
6. Ensuring that the company meets all ESRD regulatory requirements and the practice of continuous Quality Assessment

7. Hiring, training, evaluation of skills and overall performance, disciplinary action of all patient care staff for assigned areas of responsibility

8. Responsible for all employees receiving appropriate training according to policy, including training to ensure ongoing compliance with all risk management initiatives and quality performance measures.

9. Responsible for addressing and acting on adverse events and action thresholds
10. Ensure all policies and procedures are communicated to and implemented by dialysis personnel
11. Ensuring the integrity of the medical records and other administrative / operation records
12. Responsible for compliance with all data collection and auditing activities
13. Oversee and ensure the integrity and safety of the facility water systems where applicable
14. Address patient concerns and questions regarding dialysis care provided
15. Ensures completion of patient care plans through organized meetings of the healthcare team to discuss patient plan of care and to resolve patient issues
16. Daily oversight of patient census and staff schedule to maximize financial efficiencies
17. Responsible for coaching, developing and overseeing performance of dialysis personnel
18. Directs, orients and trains new RN and other personnel in leadership, management of the facility staff, policy and procedure and QAPI responsibilities. Provides counseling for all staff members, offering support, encouragement and professional growth as appropriate

19. Interviews dialysis personnel prospective candidates and administers employment offers
20. Completes timely performance appraisals and sets annual goals for dialysis personnel
21. Ensures compliance with Human Resources policies and procedures
22. Responsible for implementing and driving teamwork within the assigned area and within each facility
23. Ensure a strong communication process between dialysis personnel and all support personnel at assigned locations.
24. Responsible for strong Medical Director and physician relationships
25. Review and approve timesheets, exception reports and mileage expense forms of all assigned personnel
26. Ensure patient and treatment charge information and other related data are accurate and submitted to the corporate office in a timely manner
27. The ability to be physically present onsite.

Other Duties as Assigned

2. Able to work overtime with little or no advance notice
3. Actively participates in leadership and Quality Assessment Process Improvement (QAPI) programs and meetings
4. Attends all required in-services and training programs
5. Understands and follows patient and employee policy and procedure

PHYSICAL REQUIREMENTS

1. Vision adequate to perform essential duties and responsibilities of position
2. Physical requirements can vary. However, in general, the position requires the following physical activities: o Ability to lift minimum of 1 pound to a maximum of 35 pounds unassisted, and able to perform repetitive standing, sitting, stooping, walking, stretching, reaching, be able to stand on feet for extended periods, and use full range of body motions

Exposure Risk Level III: Lowest Risk
Positions in which no required tasks involve no greater exposure to blood, body fluids or tissues than would be encountered by a visitor. The normal work routine involves no exposure to blood, body fluids or tissues and the worker can decline to perform tasks which involve a perceived risk without retribution.
