Personal Care Aide and Skills Instructor

Affinity Family Care is currently hiring for an immediate position providing direct care and teaching life skills to an adult male (see description below). This position is 1:1 in his home and community.

**Transportation in your own vehicle is mandatory and required. Applicant must have a reliable vehicle with Air Conditioning and Heating.**

PAY: - Pay is $20/hour Enhanced Rates for all services.

Position Hours: Hours are Monday - Friday 6:30am to 4:30pm. Position is guaranteed full-time with paid sick days.

Location: Primary: Warner and Signal Butte in Mesa and Secondary: Crismon and Baseline in Mesa. Applicant must live within 15 miles of this location.

About the Position: This position is working 1:1 with a non-verbal adult male who is 23 years, has epilepsy, as well as a cognitive developmental disability.

Position requires development of a daily program that provides him a sense of purpose by developing creative learning activities that increase his intellectual, emotional, physical and social growth. Daily tasks include physical/exercise activities, community outings, life skills tasks involving home and community outings to develop independent life skills, like ordering and paying for food via touch screen, being read aloud to, creative art activities, exposure to meal prep, cooking in 1-2 step tasks and self-help skills as needed. Individual also needs assistance with meals, toileting to include wiping, daily hygiene and sanitization protocols for immune system. Position is PERMENANT and LONG TERM.

Requirements (PLEASE READ ALL):

• Must be able to pass a full background and have no criminal charges after the age of 18.
• Must be able to provide a minimum of a 2-year commitment.
• Must have a minimum of 2 years working in an educational setting or caregiving role.
• Must be dependable and punctual. We will not accept calling out as individual needs stability and consistency.
• Must be a leader and role model.
• Must be able to establish and follow a routine.
• Must have strong communication skills.
• Must have experience with behaviors and implementing preventions and supports to increase safety. This experience can be professional or personal life experience. We are willing to train.
• Must have dependable transportation with Air Conditioning and Heating and a willingness to transport and participate in community activities.
