Description : OVERVIEW

Provides general medical care / examinations, diagnosis, treatment and at times follows up care or planning to patients in the hospital, the emergency room, Rural Health Clinic under the directions / collaboration of the Medical Director / physician.

ESSENTIAL JOB FUNCTIONS :

  • Performs initial and periodic medical history, physical examinations, and daily assessments.
  • Records findings of examinations, assessments and care plans.
  • Performs minor medical and / or surgical procedures.
  • Writes appropriate prescriptions and administers medications.
  • Orders and interprets daily laboratory and radiology examinations.
  • Determines treatment plan for patients.
  • Documents of all patients encounters records observations, interventions, and actions in a medically appropriate manner on the patient's medical record.
  • Acts as a patient advocate for the population served.
  • Attends, participates and contributes to group / practice meetings and development of policies and procedures when requested.

RESPONSIBILITIES / KNOWLEDGE / SKILLS :

  • responsible for admitting patients to the hospital from the emergency department, outpatient clinics, or other healthcare facilities. They conduct thorough initial assessments, including medical history reviews, physical examinations, and diagnostic tests, to formulate a diagnosis and treatment plan.
  • oversee the ongoing care and treatment of hospitalized patients, coordinating with other members of the healthcare team, including nurses, specialists, therapists, and case managers. They monitor patients' progress, adjust treatment plans as needed, and address any complications or changes in medical status promptly.
  • provide medical consultations to other healthcare providers, including primary care physicians, specialists, and surgeons, regarding the management of complex medical conditions, perioperative care, and medical clearance for procedures or surgeries.
  • serve as the central point of contact for coordinating patient care among various healthcare providers and services within the hospital. They collaborate with specialists, ancillary services (such as radiology, laboratory, and pharmacy), and support staff to ensure timely and comprehensive care for patients.
  • educate patients and their families about their medical conditions, treatment options, and discharge plans. They address patients' questions and concerns, provide guidance on medication management, lifestyle modifications, and follow-up care instructions, and facilitate smooth transitions of care from the hospital to other healthcare settings.
  • participate in quality improvement initiatives and patient safety protocols within the hospital, working to enhance the quality of care, reducing medical errors, and improving patient outcomes. They may contribute to clinical research, guideline development, and performance improvement projects aimed at optimizing hospital-based healthcare delivery.
  • coordinate discharge planning and transitional care services for patients leaving the hospital, ensuring that patients have appropriate follow-up appointments, medications, and support services in place to continue their recovery and prevent remissions.
  • Requirements :

  • High School Diploma
  • Graduation from an accredited NP or PA program. Must be Acute Care NP or Physician Assistant. Current license to practice as a NP or PA in North Carolina.
  • Current prescriptive privileges and DEA license required. BLS for healthcare provider from AHA required.
  • Nurse practitioners are required to have a master’s degree or doctorate in nursing practice with ACNP, AGACNP board certification.
  • Physician Assistants with certification from the National Commission on Certification of Physician Assistants preferred. Current or pending CAQ certification preferred.
  • Demonstrates appropriate critical thinking and interpersonal skills when working in a fast paced, high acuity work environment. Able to perform thorough history and physical assessment. Able to work with a multidisciplinary team
