Job Purpose

Hospice aides provide for the personal care of the terminally ill patient and perform related tasks in the patient's home according to the patient's plan of care as prescribed by the patient's physician.

Core Duties

1. Provide care to ensure continual assessment and personalization of each patients and familys needs from admissions throughout the hospice stay while collaboratively working with others to achieve patient, personal, and Company goals.

2. Understand and abide by state / federal / Medicare / ACCREDITATION hospice regulations, HIPAA / PHI regulations, Office of Civil Rights, and Company policy and procedures.

3. Under the written and / or verbal direction of the RN, the CNA performs the following duties :

  • a. Provide bed bath, shower, or tub bath as appropriate;
  • b. Provide appropriate care of skin, hair, mouth and feet (excluding nail care), nail care for hands will be provided only as directed by the RN;
  • c. Provide good body alignment, positioning, and range of motion exercises for non-ambulatory patients;
  • d. Assist patient in and out of bed and with ambulation (including gait belt, sliding board, Hoyer Lift, E-Z Stand) with family or facility staff assistance as indicated;
  • e. Assist patient with use of bedpan, urinal, commode and bathroom;
  • f. Assist with routine catheter care and enemas according to the plan of care

  • g. Assist with feeding and dressing patients;
  • h. Monitor and record temperature, pulse, respirations and blood pressure, as directed by the RN, and monitor patient needs. Changes in the patients condition and needs will be immediately reported to the RN;
  • i. Change simple dressings as directed by the RN;
  • j. Remind patient in taking medications. Assist patient with pre-measured medications that are self-administered by the patient;
  • k. Perform various housekeeping duties related to patient care;
  • l. Perform simple procedures as an extension of therapeutic services as delegated by the RN and in accordance with the plan of care;
  • m. Attend patient care conferences as requested by the RN or management.

    n. Pick up from the office medical supplies to meet patient needs; and

    o. Provide homemaking services.

    4. Responsible for charting visits in to electronic medical records, assuring accuracy, completeness and compliance.

    5. Identify and address comfort care needs within scope.

    6. Support and instruct the patient, family, and caregiver.

    7. Maintain the dignity of the dying patient.

    8. Support the patients and family's unique spiritual and cultural beliefs.

    9. Provide holistic, family-centered care across treatment settings to improve the quality of life.

    10. Consult and collaborate with the interdisciplinary team and others involved in the patients care.

    11. Provide care consistent with State, Federal, ACCREDITATION, Medicare, and NHPCO standards of practice for hospice.

    12. Complete all charting within 24hrs of patient visit.

    13. Participate in continuing education programs. Maintain at least the minimum amount of approved training, education and in-service requirements according to hospice policies and State and / or Federal regulations and standards.

    14. Communicate with coworkers and management in a productive, respectful, and factual manner. Being mindful of tone, body language, and words used.

    15. Represent the Company in a professional manner by being mindful of behavior and what is communicated online or in person written or verbal format (ex. meme), wearing business casual / scrubs, being compliant, and be great at your job.

    16. Hospice is not a secret, but how the Company operates is and confidentiality will help with future success. Must maintain confidentiality.

  • Minimum Requirements

    Education & Experience

    1. High school diploma or GED.

    2. CNA Hospice experience 0 to 4 years.

    Knowledge, Skills, and Abilities

    1. Read, write, and speak English.

    2. Work independently and under the direction of the RN and management.

    3. Able to provide services within the scope of the CNA role as directed by the RN and management, and without the need of direct or observed supervision or assistance.

    4. Drive self to and from patient visits and the office.

    5. Experience with tablets and smart phones.

    Certifications and Licenses

    1. Certified or licensed by the Board of Nursing or designated agency.

    2. CPR certified

    3. Valid Driver license

    4. Auto Insurance that meets minimum state regulations.

    Physical Requirements

    Continuously : in a changing work environment (wet / dry / hot / cold), operating technology, completing charting, and in.

    Frequently : sitting, standing, walking, pushing and pulling up to 50lbs, and driving a vehicle.

    Occasionally : pushing and pulling over 50lbs and up to 100lbs, squatting, bending, kneeling, reaching, twisting, operating in tight spaces.

    Rarely : Pushing and pulling over 100lbs, crawling, climbing, walking on rough ground, exposure to dust / fumes / gases, being near moving equipment, operating medical equipment

    Never : Working from heights or working underground.

    Working Conditions

    Job requires flexibility in schedule to meet patient needs. Mainly working during daytime hours, but overnight or on-call hours may be necessary. Weekends and holidays may be necessary. Position requires travels causing exposure to temperature changes, weather changes, and traffic changes. Hospice can also expose staff to stress, depression, burnout, fatigue, long hours charting, and hauling supplies in the car.

    Promote Culture of Safety : Demonstrates commitment to personal responsibility and value for safety. Communicates safety concerns and uses and promotes safe behaviors based on training and lessons learned.

    Subject to and expected to comply with all Company policies and procedures, state and federal and Medicare and ACCREDITATION guidelines.

    Development : Engage in on-going self-development, identify and address skills and / or knowledge gaps, seeks opportunity to solicit feedback and exchange ideas, develop and achieve goals, and take on new responsibilities.

    Organization : Demonstrate understanding of manager and Company goals and works to try and make them successful. Flexibility is required as healthcare constantly changes.
