Key Responsibilities :
- Oversee the training and education of RN Care Managers, ensuring they are proficient in advanced clinical skills such as tracheostomy care, ventilator management, wound care, wound VACs, IV infusions, and more.
- Conduct comprehensive assessments of clients with high acuity needs to develop individualized care plans.
- Provide competency evaluations for RNs delivering care to high-acuity clients, ensuring adherence to best practices and regulatory standards.
- Lead the implementation of clinical protocols and procedures to maintain the highest quality of care.
- Serve as a clinical resource and mentor for the nursing team, promoting continuous professional development.
- Collaborate with the interdisciplinary team to coordinate care and optimize client outcomes.
- Maintain accurate and detailed documentation of training activities, client assessments, and competency evaluations.
- Ensure compliance with all federal, state, and local regulations governing home care services.
