We are currently seeking a per diem Registered Nurse Supervisor to join our rapidly growing team to oversee Suffolk County. At Affirmed Home Care, we believe in compromising nothing when it comes to providing exceptional, client-centered care. This philosophy extends not only to our clients, but to our staff as well.
As the RN Supervisor, you will be responsible for developing and maintaining client-specific care plans, ensuring these plans are followed, and providing for the client’s ongoing needs in a private pay, concierge level home care. This role requires 100% fieldwork for a small caseload of clients demanding great attention to detail.
This is a per diem position with an hourly rate ranging from $55.00 to $150.00 per hour.
QUALIFICATIONS:
- Current NYS RN license in good standing
- BSN preferred
- Minimum of two years recent clinical experience within a Home Care setting
- Strong assessment and organizational skills
- Strong time management skills
- Prior supervisory experience with field staff
- Proven track record of working independently
- Authorization to work in the US and up-to-date COVID vaccination required.
PRIMARY RESPONSIBILITIES:
- Ensuring the highest quality of client care
- Maintaining care plans
- Obtaining necessary physician orders for home care cases as required by State regulations
- Complete and submit assessment paperwork
- Reporting changes in the client’s medical or mental condition to the attending physician and the Agency’s team involved in the client’s care
- Supervising and supporting the home care team
- Striving to improve the lives and sustainability of our clients by supporting their autonomy in the home environment
