Job Overview
The Health Promoter Program aims to enhance the physical and mental health of older adults, focusing on immigrants and refugees. It increases access to health and mental health services, community resources, and promotes healthy behaviors, monitors medications, and provides health education. This initiative is a collaboration between the Afghan Elderly Association and the City of Fremont.
Responsibilities
- Manage a caseload of 20 to 35 clients, contingent upon the number of approved working hours.
- Conduct home visits, meet clients at appointments, in-office consultations, and maintain communication via phone or video conferencing.
- Assist and advocate for clients in obtaining and maintaining access to services such as CAPI, GA, CalFresh, IHSS, housing assistance, Medi-Cal, Medicare, Social Security, legal services, immigration services, transportation, and other community programs. Responsibilities include making calls, scheduling appointments, attending meetings, completing necessary paperwork, and providing additional support as needed.
- Perform health, mental health, and safety screenings.
- Offer emotional support to alleviate stress and promote healthy lifestyles.
- Provide translation assistance to facilitate access to programs and services, as well as during medical and social service appointments.
- Arrange transportation or assist clients in securing transportation services.
- Attend the weekly Healthy Aging Program and fulfill assigned responsibilities.
- Deliver health education to clients and the broader community.
- Provide information and assistance to clients as needed.
- Complete all documentation accurately and in a timely manner.
- Engage in outreach and client recruitment efforts.
- Participate in Health Fairs.
- Understand and adhere to HIPAA and privacy regulations, ensuring client confidentiality.
- Maintain professional boundaries in all interactions.
- Work effectively with individuals of all genders.
- Perform other duties as assigned.
Requirements
- Fluent in Dari and English.
- Proficient in Microsoft Word, Excel, Outlook, and Teams.
- Skilled in using an iPad and computer.
- Knowledgeable about CAPI, GA, SSI, IHSS, Medi-Cal, Medicare, CalFresh, housing assistance, immigration issues, local community resources and programs, and other governmental programs.
- Comfortable working with clients who may experience mental health challenges and/or housing instability.
- Flexible and adaptable to changing environments.
- Possess strong organizational skills and demonstrate reliability and dependability.
- Ability to collaborate effectively as part of a team.
- Valid Driver’s License.
- Bachelor’s Degree, Associate’s Degree, or relevant experience.
