We are a "Qualified Organization" that employs Waiver Support Coordinators as providers for the State of Florida's Agency for Persons with Disabilities. In a nutshell, we coordinate services for people with developmental disabilities.
Compensation:$25 - $30 hourly
Responsibilities:Your job duties would require you to:
- Interview assigned consumers, individuals, their families, and other responsible parties. Assist in completing necessary documents such as a Support Plan and uploading them into the State's computer system.
- Prepare monthly case notes that sufficiently document the Support Coordination services by the due date.
- Monitor services to ensure that the terms of the authorization are being fulfilled by the contracted providers by maintaining face-to-face contact as required with consumers, guardians, and providers.
- You are expected to work 50 hours per month - 30 hours per month will be in one of our office locations in Broward, Palm Beach, or Fort Pierce - 20 hours will be in the field. It may include nights or weekends - we will work with you to develop the schedule that works for both of us.
- Bachelor's Degree in Social Services, Criminal Justice, or Human and Health Services and related fields.
- Two years of experience in social services.
- Must live in South Florida, with a valid driver's license and insurance willing to drive in Palm Beach and the Treasure Coast.
- Demonstrated desire to help others in need.
- Must pass a Level 2 Background Check.
The African-American Advocacy Center for Persons with Disabilities ("AAACPD") mission is three-fold.
First, we are to provide awareness within the African-American community of disabilities and the resources necessary to overcome these challenges.
Second, we will utilize our advocacy skills to provide a voice to African Americans and others who have a need for our legal services.
Last but not least, AAACPD will provide, or work with partners, to provide direct services to children and adults affected by any type of developmental disability. We believe that through awareness, advocacy, and access, persons with disabilities can improve their well-being and become productive members of our community.
AAACPD works in conjunction with the Law Offices of E.F. Robinson, P.A. which provides legal services to help families turn the page in their Book of Life to the next chapter full of positive and new experiences.
