Case Manager
Case Manager Position Summary :
This position is for Case Manager that provides care for adult clients in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; also; assumes the responsibility for coordination of care.
Case Manager Job Functions, Responsibilities and Duties:
- Completion and on-going assessments of the AFC qualified setting (member and caregiver application)
- Obtaining a social history and conducting a psychosocial assessment and evaluation.
- Participation in the selection, screening, training, and supervision of AFC caregivers in conjunction with the AFC registered nurse.
- Monitoring the psychosocial status of all members to ensure that all needed AFC services is being provided.
- Review all caregiver logs on each visit and more frequently if needed.
- Conduct the on-site visit with client and caregiver monthly or more frequent visits as the client's health/psychosocial status warrants.
- Completion of a psychosocial progress note with each visit or encounter.
- Planning for and implementing discharges from the AFC Program.
- Conduct an orientation for each AFC caregiver before the caregiver begins providing care and providing on-going training to AFC caregivers on health and aging.
- Assist with obtaining information and accessing other health-care and community services
- Reviewing and documenting the fire and safety procedures for the qualified setting
- Review every 6 months the suitability of the qualified setting
Case Manager/ Job Qualifications :
- Must have a Bachelor's Degree, A Social Worker's license from the Massachusetts Board of Registration in Social Work and have at least 2 years of recent experience in working with elderly or disabled clients or a Bachelor's Degree and 2 years clinical experience in the care of elderly or disabled persons.
- Case Manager must be in good physical and mental health and display mature and accurate decision-making ability. Mandatory physical exam which includes a Mantoux is required. Follow up physical exams required every 2 years to ensure that the nurse is in good medical and emotional health.
